Campus Subpoenas
Risk Services is the authorized entity on campus responsible for receiving, documenting, and coordinating responses to subpoenas relating to campus matters.
Please include a $15.00 check payable to “UC Regents” at the time of service to cover the University Record Request Fee, also known as a Witness Fee.
Note that Risk Services does not accept walk-ins. If you require in-person assistance,
kindly schedule an appointment in advance. Mailing is preferred.

A subpoena is a legal document that commands a person or entity to testify as a witness at a specified time and place (at a deposition, trial, or other hearing), and/or to produce documents or other tangible objects in a legal proceeding. Subpoenas are time-sensitive with court-imposed deadlines.
If you are contacted by an agent of service, that individual should be directed to our office for all campus subpoenas. If you happen to receive a subpoena by mail or in person, please note the date and time of receipt on the copy received and submit a General Inquiry to Risk Services.

The Office of Risk Services must respond to campus subpoenas within certain legally prescribed deadlines, so your collaboration with Risk Services is essential and valued.
UCPath only accepts the garnishment order by fax or mail from the withholding agency or the employee's location administrator. Please forward all Earnings Withholding Orders/Wage Garnishments to:

Upon receipt of the modified or release order from the withholding agency, UCPath will adjust or remove the wage garnishment order accordingly.
